Please note these forms are in only two colors Red and black. Also the form is incorrectly label as 1240 DT instead of 1240T.

The further away you are from us in Alberta, and the bigger the order, the more likely we will need to reach out to you to cover the fuel surcharge before we can ship your order.

For example, the fuel surcharge for a box of five bundles to Ontario or Nova Scotia in January of 2024 was about $8 plus tax. The fuel surcharge for a box of 12 bundles to Ontario or Nova Scotia in January of 2024 was about $14 plus tax.
